An exciting opportunity has arisen to be part of the Gosport Integrated Community Care Team.

This is a full time Community Nursing position which is designed to contribute to avoiding hospital admission and maintaining patient's independence in their own homes throughout the Gosport locality. This achieved by both routine visits but also responding to Urgent Crisis Response calls which are a key aspect of the role going forward. The UCR process is a new initiative in which we work alongside SCAS, offering acute visits with other members of the MDT, and using Community based skill set to try to reduce the need for an Ambulance to attend and to prevent the Patient being admitted.

This post offers the opportunity to work closely with all members of the ICT both in primary, secondary care and adult services as well as working closely with care home staff.

The successful candidate would be required to actively participate in ICT/MDT meetings along with other members of the team and the wider MDT disciplines, coordinating a plan of care and working within a scope of practice autonomously as a registered practitioner. They will be expected to be involved in care planning and identification of person centred care delivered in the home setting. Ideally the successful candidate will possess excellent communication skills and be competent in working together with the larger multidisciplinary team.
