Reablement Worker needed with London Borough of Hammersmith and Fulham

Start Date - ASAP

Location - Hammersmith

Duration - Initially for 3 months

Hours per week - 36 hours per week

Salary - £19.45ltd per hour

Description:

• As a reablement worker you will be supporting clients with personal and practical care tasks whilst maintaining and developing their social contacts.
• To provide written and verbal reports regarding the activity undertaken and progress made against goals during supervision, at reviews and when required, utilising electronic and paper based records
• To work in partnership with other teams and agencies within the borough to maximise reablement potential and support the transition into and out of the Community Independence Service.

What is required?

• Previous experience of working with vulnerable Adults within the community
• Educated to GCSE Level Grade A-C in Maths and English
