Our Programme Workers at Change Grow Live play an integral part in delivering our Foundations of Rehabilitation programme which is a platform for the delivery of desistance-oriented interventions and support. We have based this programme model on hope, resilience, and compassion.

We are currently recruiting a Through the Gate Coordinator to work as part of the newly implemented Southwest Prisons service based in HMP Exeter. This role involves holding a caseload of service users that are ready for release, working alongside priuson stakeholders to enable an effective transfer of service user treatment.

Salary: £26,393 - £30,316 dependent on experience
Responsibilities:
About the role:

  • Work intensively with service users for 12 weeks prior to and post-release
  • Tailor group work sessions preparing for release to both short term and long-term
    populations – as per prison client cohort
  • Review personal goals, preparing service users for release (e.g. what to expect on day
    one of leaving the gate)
  • Work closely with CGL Teams, Healthcare, Prison partners and External Support
    Agencies to ensure effective coordination of Release Planning
  • Contribute to risk management processes, including prison ACCT process as
    appropriate
  • Provide accurate and up to date harm reduction advice to service users, including
    delivering training in the use of Naloxone
  • Support patients in organising their appointments, including attending and advocating
    on client’s behalf where necessary

About you:
  • Experience of working within a prison/criminal justice setting is desirable but not essential
  • Have a strong understanding of substance misuse issues
  • Be an excellent communicator, both verbal and written with good IT skills
  • Proactive and able to work on own initiative with excellent time management and prioritising skills
  • Have a good working knowledge of mental health interventions, services, and good practice
  • Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
  • Ability to manage change successfully in a way that prioritises the needs of service users
  • Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
  • Be able to travel as required, including where public transport is not available
