Job Description
The overall purpose of the Structural Heart Valve Disease team is to care for patients via the percutaneous programme that includes Transcatheter Aortic Valve Implant (TAVI), MitraClip, Tendyne, and TriClip as part of National, Trust and Division structure.
You will be managing the patient pathway to ensure a timely and efficient pathway through the percutaneous programme. The role will work within a dedicated specialist team to ensure that the patient journey is coordinated and streamlined and administrative processes are revolved around the patient and their individual needs. The role requires sufficient knowledge in various IT systems, with excellent communication skills both verbally and written. Applicants should be able to work effectively within a team, but also work independently and be able to use their own initiative. The patient pathway is complex and requires excellent organisational skills.
- To support the triaging of referrals by liaising with key clinical/non-clinical staff
- Manage patient communications both via telephone and through trust systems to create letters and emails
- To act as a point of contact for queries from clinical and non-clinical staff
- Liaise with clinical and non-clinical teams across the Trust and other referring hospitals, via telephone and email
- Escalate patient queries, capacity problems and general issues to the role’s line manager or relevant clinical staff as appropriate
- Communicate changes to clinics and appointments to patients as instructed, often at short notice
