Lancashire and South Cumbria NHS Foundation Trust is currently seeking a highly skilled and motivated Band 6 Qualified Social Worker to join our dedicated team for Multiple Vacancies available. Position: Band 6 QSW Care Co-Ordinator
Location: Lancashire and South Cumbria NHS Foundation Trust
Salary: up to £37 per hour (depending on level of experience)
Responsibilities:
Coordinate care and support services for patients
Collaborate with a multidisciplinary team to develop and implement care plans
Ensure timely and appropriate delivery of care interventions
Liaise with external agencies and community partners to facilitate smooth care transitions
Provide guidance and support to patients, families, and carers regarding available services
Conduct assessments and reviews to monitor patient progress and adjust care plans as needed
Requirements:
Relevant qualification in social work or a related field
Previous experience as a Band 6 QSW Care Co-Ordinator or similar role
Excellent communication and interpersonal skills
Strong understanding of person-centered care and care coordination principles
Ability to work effectively within a multidisciplinary team
Valid driving license and access to a vehicle for work purposes
