An exciting opportunity has arisen to lead this new and dynamic integrated multi-disciplinary team, SCOPE working with individuals with a learning disability and/or autism who have complex care requirements. The team will be supporting individuals from the age of 14 years and upwards at different life stages, from transition from school into adulthood, moving to their own accommodation and beyond into older age.

The team will be made up from disciplines from both Health and Social Care, such as social workers, Outreach Workers, Psychology and aligned to NHS services that include Speech and Language Therapist, Physio Therapy, Dietetics, Psychiatry and Community Learning Disability Nurses. The team will link with local partners such as Education and Children’s services, Housing, other Health Services and 3rd Sector.

SCOPE will be leading in developing locally based provision, which will improve the lives of those with complex care requirements as well as their families and carers. The team will have a whole person approach and will embed the principles of positive behavioural support into the delivery of provision.

We are looking for a dynamic person who is passionate about working with individuals who have a learning disability and/or autism and has an understanding of the complexities of this client group. Who is wiling to embed the work which has already commenced and lead the continued development of this team, placing those who access the service at the heart. They will also be involved in the ongoing integration work within the partnership, working alongside colleagues in the Learning Disabilities Health Service.

Requirements

We are looking for individuals who have experience of working with vulnerable people in the community, with in-depth knowledge of Learning Disability and Autism. Having an understanding of individuals who are vulnerable or at risk is also essential, including oversight of adult protection concerns which arise within this client group.

  • Knowledge and expertise to shape strategic direction of Learning Disabilities and Autism activity to ensure effectively delivery of services
  • Knowledge of a range of public, independent and voluntary sectors and the social contexts of these fields, to ensure service users are supported and share knowledge and best practice
  • Interpersonal skills to build and maintain relationships with a wide range of partners and communities to ensure personalised and customer focused service delivery in with line National Outcomes
  • Communication skills to explain wide ranging complex and contentious information with a range of audiences exercising negotiating and persuading skills
  • Analytical skills and understanding to asses and analyse complex information, problems/situations and develop strategies or plans
  • Planning and organisational skills to lead others, considering the implications now in the longer term, to support work area objectives and National Outcomes
  • Ability to assess, create, oversee and implement appropriate interventions for the wellbeing and/or learning needs for client/service users

You will have a Social Work Degree, CQSW or DipSW.

You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.
