We have an exciting opportunity for a Medicines Management Pharmacist to join our busy Birmingham service, based in Park House. The new post holder will provide comprehensive medicines management support to ensure safe and effective use of medicines across Park House and will work with clinical and operational colleagues to promote best practice in relation to medication use.

Hours: 11hrs per week
Salary: £54,739 - £62,692 dependent on experience (pro rata for part-time)
Responsibilities:
About the role:
- Provide medical information to both internal and external healthcare professionals
- Assist with pharmacovigilance related queries
- Be the point of contact for nursing and clinical staff regarding medicine management issues
- Provide patient education on medicines use to inpatients and on discharge
- Be able to identify high-risk drugs, significant drug interactions and those drugs that require therapeutic drug monitoring
- Ensure that Medicines Reconciliation is undertaken for every service user admitted as an inpatient
- Support colleagues with adherence to Formulary and cost effective prescribing
- Ensure all medication recording is accurate and medication administered is within required parameters and handled in accordance to manufacturer guidance
- Maintain an up-to-date formulary in line with current guidance
- Develop, review and update PGDs for effective service delivery
- Create, review and update organisational policies, procedures and guidance documents to ensure good medicines management governance
- Undertake Medicines Management Audits and providing written feedback via Medicines Management reports to ensure compliance to regulatory standards and organisational policies / procedures
- Review and analyse medication related incidents; providing support with incident management and sharing learning from incidents to improve practice within clinical services
- Promote a learning and development culture throughout the organisation to maintain the highest standards of care
- Provide training to staff via a range of teaching methods including online presentations and webinars to ensure competency in key medicines and prescription related areas
About you:
- Current registration with GPhC
- A minimum of 2 years’ experience as a registered pharmacist within the UK
- Knowledge of Inpatient/ Care home settings
- Be able to demonstrate a wider understanding of pharmacy practice and the workings of the NHS, including prescribing and medicines management
- Computer literate with Microsoft packages; proficient in the use of Microsoft Excel
- Experience of developing clinical protocols and PGDs
- Experience of delivering training.
- Experience in the substance misuse field
